Need to create a sub-task for the template ServiceCatalog.Request.Template

Hi Techies,

Would like to know the possibility of Creating the subtask for the readymade template given by Symantec named "ServiceCatalog.Request.Template".

I'm planning to customise my own ServiceRequset form with my own Prefix along with the Subtask creation.

Struckup now in creating the Subtask for the ServiceRequest.

Please help me with your ideas or approach.
