Altering the customer satisfaction survey form in 7.5

Hi All,

Does anyone know where i can find instrcutions on how to alter the Customer Satisfaction survey form in 7.5. I have looked but can only find instructions for 7.1 and they do not carry over to 7.5. I am looking to customise it and add some images. i would also like to know how to change the default link on the survey email so that it takes the customer directly through to the survey and not to the incident page.

Any advice greatly appreciated.
